Submit a copy of the applicant's shot records.

Submit the Elementary Health Form.

Submit a copy of previous report card, if applicable.

Submit the Request for Records Authorization, if applicable.

Submit Teacher Recommendation Forms and a Principal Recommendation Form, if applicable.
